Tweaks to login page template (Closes FS#771) [alk]

  - Remove hover-popup on "Permanent connection" checkbox
  - Always display the warning text previously contained in this hover-popup
  - Tweak the submit button's label
  - Capital first letter for the "Lost password" link
  - Move error message when login fails to a more visible location
    - was at the very bottom of the page
    - is now located right before the login form
